Anemia is a nutritional problem that often occurs in adolescents, especially adolescents who live in dormitories. Anemia is a decrease in the quantity of red blood cells in the body or the amount of hemoglobin is below normal limits. Food intake that is less varied with less quantity and frequency so that it can affect the nutritional status of adolescents. Abnormal nutritional status, very active physical activity can affect hemoglobin levels in the blood. Adolescents who experience anemia can disrupt learning concentration, inhibit growth and development, and result in weak immune systems so that they are more susceptible to disease. The purpose of this study was to determine the relationship between food intake, nutritional status, and physical activity with the incidence of anemia in adolescents at the Ar-Rahman Islamic Boarding School in Palembang City. This research uses a cross sectional study design. The sample in this study were teenagers aged 13-18 years, totaling 130 people. The sampling technique is Stratified Random Sampling. Data analysis techniques are univariate and bivariate. Data analysis using Chi-Square test and Simple Logistics Regression. Data is presented in tabular form accompanied by narration. The results of bivariate analysis showed that there was no significant relationship between food intake (deficiency of vitamin A, deficiency of vitamin B6, deficiency of vitamin B9, deficiency of vitamin B12, deficiency of vitamin E), poor nutritional status, and physical activity with the incidence of anemia (p>0,05) in adolescents. Meanwhile, there is a significant relationship between food intake (deficiency of protein, deficiency of iron, deficiency of vitamin B2 and deficiency of vitamin C) with the incidence of anemia (p
