WHO recommends that children be breastfed only breast milk for at least 6 months, thus iron adequacy for infants will be fulfilled provided that the mother does not experience iron deficiency. Iron deficiency in breastfeeding mothers will reduce milk production and reduce the quality and quantity of breast milk and will have an impact on the iron content in the mother's milk. This study aims to determine the effect of iron deficiency on breast milk iron levels in breastfeeding mothers in the Seluma District. This research is research cross-sectional and held in 4 working areas of Puskesmas in Seluma Regency. Respondents were 52 breastfeeding mothers who were taken using the technique of Purposive Sampling. Venous blood was taken to measure serum iron levels and breast milk samples were taken to measure breast milk iron levels. Iron levels were measured using the spectrophotometric method. Characteristic data was taken through a questionnaire, and data on iron intake was taken using SQ-FFQ questionnaire. Data were analyzed using statistical software in univariate and bivariate forms. Based on serum iron measurements, it was found that the majority (69.2%), or as many as 36 breastfeeding mothers experienced iron deficiency. The results of measuring breast milk iron levels showed that there were 27 respondents or the majority (51.9%) of breastfeeding mothers had low breast milk iron levels. The results of measuring iron intake found that 78.8% of breastfeeding mothers had insufficient iron intake. Test results chi-square for serum iron levels and breast milk iron levels obtained p = 0.022; PR=2.556; CI=1.056-6.183. Iron deficiency in breastfeeding mothers affects breast milk iron levels. Breastfeeding mothers who experience iron deficiency are at risk of 2.5 times having low breast milk iron levels.
