Lockout/tagout is an activity related to the use of safe energy sources in a job to refer to all work situations, including industry where there are certain and special provisions. The entire system must be properly constructed and properly maintained. Any work on the system must be performed in a secure manner. Energy source equipment working in a hot environment must be designed and built to deal with it. As an effort to prevent accidents, workers need a system that can protect them from the release of hazardous energy as an effort to prevent work accidents caused by this hazardous energy, namely safety procedures.Lockout & Tagout (HEART). The mechanic is obliged to implement the procedureHEART when carrying out maintenance and repair work as an effort to prevent work accidents. The purpose of this study is to conduct a special evaluation of aspects that support the implementation of Lock Out Tag Out (LOTO) as an Effort to Prevent Work Accidents in the Care and Maintenance Department of PT. Bukit Asam Tbk. This study uses qualitative methods with data collection techniques by means of interviews, observation and document review. The informants for this study consisted of 4 key informants and 4 regular informants. The data validity test used was source triangulation, data triangulation and method triangulation. The results of the study show that there are conditions where the existing security labels do not meet the requirements for durability and can withstand extreme working conditions. Based on this study it can be concluded that the implementation of LOTO in the care and maintenance department of PT. Bukit Asam Tbk has been running quite well, but there are still labels that do not meet the requirements optimally.
