Background: Irrigation of the root canal in the apical third is a difficult part because there are additional lateral and furcation canals, so it needs an additional technique to clean the apical third of the root canal. The ultrasonic agitation technique uses a vibrating instrument to produce a penetration that reaches the dentinal tubules with the same reach in the coronal, middle, and apical thirds. The requirements for ideal irrigation are non-toxic, can dissolve the remaining necrotic pulp tissue, and remove the smear layer. Plant extracts such as Averrhoa bilimbi L can be used as an effective alternative for irrigation because they are biocompatible and non-toxic. Objective: To determine the effectiveness of Averrhoa bilimbi L leaf extract 15% as irrigation on the cleanliness of the apical third of the root canal using the ultrasonic agitation technique. Methods: The study used 20 samples of mandibular premolar roots prepared using the crown down technique and irrigated based on the groups, namely: group A Averrhoa bilimbi L leaf extract 15% with ultrasonic agitation technique, group B Averrhoa bilimbi L leaf extract 15% with non-agitation, group C NaOCl 2.5% and 17% EDTA with ultrasonic agitation, and group D 2.5% NaOCl and 17% EDTA with non�agitation. The samples were then cut longitudinally and evaluated using SEM at 5000x. The results were analyzed by the Kappa test, followed by the Kruskal-Wallis and post hoc Mann-Whitney tests. Results: There was no significant difference in the cleanliness of the apical third of the root canal area between groups A-C, A-D, and C-D, but there was a significant difference between groups A-B, B-C, and B-D. Conclusion: Irrigation of Averrhoa bilimbi L leaf extract 15% using ultrasonic agitation technique is effective for cleaning the apical third of the root canal.
