Background: Hypercholesterolemia is a condition when total cholesterol increases beyond its normal value (≥200 mg/dL). Hypercholesterolemia is the main cause of atherosclerosis in blood vessels, such as in peripheral arterial disease, myocardial infarction, and stroke. The most commonly antihypercholesterolemia drug that used in Indonesia is simvastatin. Irrational use of medicine is still the major health sector problem in the world. Irrational use of medicine causes some negative effects to patients, such as: polypharmacy, unwanted side effects, and adverse drug interactions, as well as increased medical costs. This study aims to determine the rationality of simvastatin use in hypercholesterolemia patients at the Tegal Binangun Primary Health Care period 1st July 2021-30th June 2022. Methods: Drug utilization study has been conducted at Tegal Binangun Primary Health Care from July 2022 to November 2022. There are 58 samples which appropriate with the inclusion criteria. The sampling technique is total sampling. The entire data will be analyzed using SPSS v24 in the form of a frequency distribution. Results: In this study, the characteristics of hypercholesterolemia patients who used simvastatin are mostly in the age group of 41-60 years (65,5%), women (74,1%), elementary school graduates (51,7%), and from Plaju Darat Village ( 58,6%). The rationality use of simvastatin is based on the criteria for the appropriate dose (100%), appropriate frequency of administration (100%), appropriate time of administration (100%), appropriate route of administration (100%), and appropriate drug interaction (74,3%). Conclusion: The rationality use of simvastatin is based on the criteria for the appropriate dose (100%), appropriate frequency of administration (100%), appropriate time of administration (100%), appropriate route of administration (100%), and appropriate drug interaction (74,3%).
