Coronavirus disease 2019 (COVID-19) is an infectious respiratory disease caused by SARS-COV-2. During the COVID-19 pandemic, the SARS-COV-2 virus underwent mutations. This mutation produces new variants with different expressions. The purpose of this study was to determine the characteristics of the mutation of the SARS-CoV-2 gene, Variant Delta, RSUP Dr. Mohammad Hoesin Palembang. This study is a descriptive analytic observational study with a cross-sectional design. The data used in this study were medical record documents for COVID-19 Varian Delta patients with whole genome sequencing (WGS) results at Dr. Mohammad Hoesin Palembang who met the inclusion and exclusion criteria. This study reports that mutations in patients with the delta variant of COVID-19 at RSUP Dr. Mohammad Hoesin Palembang in 2019 – 2021 has the highest percentage in the age group 19 – 59 years (85.71%) and is female (65.3%). Vaccination status of COVID-19 patient variant delta at Dr. Most of Mohammad Hoesin Palembang were patients with 2 doses of vaccination (59.2%). The location of the mutations that occur in COVID-19 patients with the delta variant is 135 sites in 20 parts of the viral protein. The part of the protein with the most mutational variations occurred in the s protein with 32 sites mutations. The most common clinical manifestation in patients with COVID-19 Variant Delta is fever (79.6%). Patients with COVID-19 variant delta at RSUP Dr. Mohammad Hoesin Palembang is divided into 4 lineages, namely B.1.617.2 (6%), AY.23 (69.5%), AY.24 (22.5%), and AY.75 (2%) with the majority aged 31-45 years, female sex and 2 doses of vaccination status. Mutations most frequently occur in the s protein with which it has a stabilizing effect, increasing the binding affinity of the protein with ACE2. The most common clinical manifestation in the sample was fever (79.6%).
