RASIONALITAS PENGGUNAAN ANTI DIABETIK ORAL PADA PENDERITA DIABETES MELITUS TIPE 2 DI RS BHAYANGKARA PALEMBANG PERIODE JULI 2010 - JUNI 2011
Background : Type 2 Diabetes mellitus is a chronic disease which is classified by the increase in blood glucose level. WHO predicted in 2004 more than 200 million people diagnose type 2 Diabetes mellitus. Indonesia is the fourth country that has a largest number of people with type 2 Diabetes mellitus, which is about 8 million people. For this reason research is needed on rational of oral hypoglicemic agent. The purpose of this study is to know the rational use of oral hypoglycemic agent to the type 2 Diabetes mellitus in hospital installation in Bhayangkara Hospital Palembang. The study is descriptive study using secondary data. Methode : The study is a descriptive of drug utilization study will be Bhayangkara Hospital Palembang began in October to December 2011. The study population was medical records of patients with type 2 DM in July 2010 - June 2011. Samples is 50 patients received oral hypoglicemic agent in the Department of Internal Medicine Hospital Bhayangkara Palembang. The result of this study are tabulated and presented descriptively. Result : The result show that 58% of type 2 Diabetes mellitus patients are male. Most of the patient (34%) are in the age of 51-60 years old. Type 2 Diabetes mellitus patients 48 % utilize oral hypoglycemic agent are categorized as normoweight, and 8 patients of those are categorized as underweight still get metformin therapy. Ali o the patients who got oral hypoglycemic agent therapy are type 2 Diabetes mellitus patients. But, there are 4 patients show a creatinin value over normal but still get metformin therapy. There is only 2 % patient show adverse effect during utilization oral hypoglycemic agent. Dose of oral hypoglcemic agent has been largely in accordance with the Standard (95,7%), but there are 2 people that have dose is not adequate. The frequency of oral hypoglcemic agent also largely been appropriate, but there is one patient who received acarbose therapy only 1 time a day so that glucose levels remain uncontrolled. More than 70% of patients have shown decrease in blood glucose levels according to therapeutic targets. Drug interactions that are synergistic with most of NSAID (23.3%) and drug interactions are most antagonistic with furosemide and corticosteroids. Concultion : There is irrationality in the provision of oral hypoglcemic agent there are 8 people underweght who received metformin, 4 person high levels creatinin enforced metformin and antagonistic interactions as much as 16 interactions.
