PDAM Tirta Musi is a regional-owned enterprise (BUMD) of Palembang City which was established to provide drinking water in Palembang City. PDAM Tirta Musi is committed to protecting employees. One form of employee protection is employee protection against potential accidents or occupational diseases. To achieve this, it is necessary to carry out hazard identification and risk analysis in the workplace. HAZOPS is one method that can be used to identify hazards or deviations in a detailed and structured manner. The purpose of this study was to analyze the K3 risks contained in the water treatment process at PDAM Tirta Musi. This research was conducted using a qualitative research design with a risk identification method using HAZOPS and conducting a risk assessment based on BS 8800:2004 by conducting field observations and in-depth interviews with informants. The results of this study found that there were deviations in the form of temperature more than normal, part of separation, no signal, no transfer, more vibration, reverse flow, pH higher and lower than normal, more addition,less addition, part of operate, part of measurement, no flow, no operate. The consequences of this deviation have an impact on the production process and also cause potential accidents and occupational diseases. The conclusion of this study is that there are 4 deviations that fall into the OHS risk, namely excess temperature and transformer oil leakage which is categorized as high risk and excessive vibration and chlorine gas leakage which is categorized as medium risk. Recommendations that can be given are to apply APAR in the intake area and chlorine gas room, provide PPE in the form of earmuffs/ear plugs, safety goggles and masks, change the layout in the chlorine gas room, make a routine schedule for checking rotameter calibration, and hasten the formation of K3
