Men who have sex with men (MSM) are defined as the key population, a high-risk group for HIV transmission. HIV cases among MSM in Indonesia have been recorded at 17.9%. This study aimed to examine the relationship between characteristics and risk behaviors with MSM incidence in Palembang City. This type of research uses combined research (mix method) with explanatory design procedures between quantitative analysis with cross-sectional design and qualitative research with a phenomenological approach as support. This study uses secondary data from SIHA 2021 through the HIV KT Report (VCT) for quantitative analysis while obtaining in-depth interviews, observations, and documentation for qualitative research. The quantitative research sample amounted to 1.708 MSM respondents, while the qualitative research sample consisted of 11 MSM primary informants and 4 key informants from the Intan Maharani Foundation and the Palembang City Health Office. Quantitative data were analyzed using univariate and bivariate analysis, while qualitative data were analyzed using thematic analysis. This quantitative study showed that MSM respondents who experienced HIV incidence in Palembang City were 4.2%. The bivariate analysis results showed that the variables of age (P=0.044) and last education (P=0.038) had a significant relationship with the incidence of HIV in MSM. The results of the qualitative study indicate that there are four themes of MSM risk behavior that can explain the research and are associated with the characteristics and incidence of HIV in MSM, including the causes or triggers for becoming MSM, openness to status with close people, MSM sexual experience, and non-sexual MSM experiences. This research is expected can provide an overview and understanding for MSM respondents, MSM informants, and the general public so that risky sexual behavior, especially sexual behavior that triggers HIV, can be prevented
