The COVID-19 vaccine coverage rate for the elderly in the city of Palembang as of April 11, 2022, the first dose is 62% to the second dose is 53%. This achievement does not establish herd immunity due to uneven vaccination rates in the elderly group. This study aims to describe the perception of the elderly group towards the COVID-19 vaccine in the city of Palembang. This study uses a qualitative research study design with a case study approach and data collection methods in the form of in-depth interviews. The sampling technique was purposive sampling, with the research sample consisting of 16 main informants, 16 key informants and 2 expert informants. The main informant samples taken were elderly people aged 50 years and over as early old age, with vaccine status which was divided into already vaccinated and not vaccinated, and the last education level was divided into elementary school graduates and high school graduates. The sample of key informants is the family of each elderly with a relationship as a child. While the sample of expert informants is the staff of the Elderly Police at the Dempo Health Center and policy makers at the Palembang City Health Office. All samples taken were differentiated based on their domicile origin, namely the Ulu region and the Ilir region. The data analysis technique uses the content analysis method. The results of this study indicate that most of the elderly in the Ilir area already have the urge to receive the vaccine, but are hampered by the condition of the elderly who have the disease. Meanwhile, a small proportion of the elderly in the Ulu area who do not have the urge to vaccinate are due to a strong belief in God, doubts and fears of the elderly about the side effects of vaccines. To minimize these obstacles, it is necessary to have a strategy from the puskesmas and the Palembang City Health Office. One of them is by approaching the elderly, through family and influential figures by adjusting existing barriers. That way, the awareness of the elderly about the importance of vaccines will improve and the coverage of vaccinations targeting the elderly in the city of Palembang will be fulfilled.
