Lismarini Clinic is one of the private clinics operated in Palembang. One of the problems that exist at the Clinic are the patients who register for treatment are recorded manually into the patient ledger, patients who seek medical treatment have no recorded data (medical records), and patients have no ability to give criticism and feedback because of lack of the media. To Overcome this problem Lismarini Clinic requires a website-based Customer Relationship Management (CRM) application, the CRM application uses the PHP programming language and uses a prototype system development method. The results from this study are to implement a website-based CRM application for the clinic, making it easier to provide information about the Lismarini Clinic, making appointments with midwives, managing patient and midwife data, pharmacy data, financial and accounting data, patient medical records, and also providing media for the patients to give feedback and criticism for the Lismarini Clinic.
