This study focuses on the knowledge and experience possessed by employees who undocumented, due to member changes that occur in the structure particular organization or program team. Therefore, knowledge management system for managing and documenting knowledge in the PTM UPTD Puskesmas Batumarta VIII work units. This study uses the Choo-Sense Making model, Choo-Sense Making is a knowledge management that focuses on three processes in development of knowledge management, that is sense making, knowledge creation and decision making. This model is used because this model can manage elements knowledge possessed and can create knowledge in new forms that are can be used to manage knowledge itself. This research resulted in a system that is useful in managing knowledge so that it is good and effective documented. This system can document structured Tacit Knowledge and Explicit Knowledge that has not been documented. Knowledge Management System This can be further developed by adding more complete features related to using knowledge sharing, and it is hoped that in the future in the choice of categories knowledge, further expanded based on user needs.
