IDENTIFIKASI MUTASI TITIK K76T GEN pfcrt Plasmodium falciparum SEBAGAI MARKER RESISTENSI KLOROKUIN PADA PENDERITA MALARIA DI SUMATERA SELATAN
Background: Malaria is an infectious disease caused by the parasite Plasmodium spp. Among the four species of the genus, Plasmodium falciparum malaria causes more serious illness with the highest mortality rate. Chloroquine has been the drug of choice for treatment (first line drug) and chemoprophylaxis since the 1940's. However, the current use against Plasmodium falciparum greatly reduced after the chloroquine resistance (CQR). CQR linked to gene mutation K76T Plasmodium falciparum chloroquine resistance transporter {pfcrt). So far study about distribution of Plasmodium falciparum resistance to chloroquine in South Sumatra was only implemented in Ogan Komering Ulu (OKU). Objective: This study aimed to evaluate a molecular marker gene pfcrt K76T point mutation associated with Plasmodium falciparum chloroquine resistance on malaria in South Sumatra. Methods: This study is a laboratory research, which is also an observational descriptive study with cross-sectional design on 30 subjects with malaria. The identification of point mutations K76T gene pfcrt of Plasmodium falciparum was done by using the Nested Polymerase Chain Reaction-Restriction Fragment Length Polymorphism (PCR-RFLP). Results: There is no amplicon with wild-type genotype KK (0%), 2 amplicons (8%) with a heterozygous genotype KT and 23 amplicons (92%) with the homozygous mutant genotype TT (92%) was identified. Distribution of allele variant alleles obtained is 2 K (4%) and 48 T allele (96%). Conclusion: Ali the isolates studied was carrying a point mutation K76T pfcrt genes with homozygous mutant genotype as the most dominant type genotype. It is proved that chloroquine is no longer an adequate drug to treat malaria in South Sumatra.
