Background: Cataract is the leading cause of blindness. In the management of cataract in Indonesia, Extraction Cataract Extra Capsular (ECCE) is widely used even though this technique has its drawbacks. These drawbacks lead to switching to phacoemulsification technique with intraocular lense implantation. Method: This study was an analytical study using a cross-sectional approach which been held in medical records at RSKMM Provinsi Sumatera Selatan. Data was analyzed with Chi-square to determine the difference visual acuity of postsurgery ECCE+IOL and Phacoemulcification+IOL. Results: 80 cases were observed. Most Incidence of visual acuity improvement of patient mature senile cataract postsurgery was found more frequently on phacoemulsification+IOL with the percentage of 90%. There was no significant relationship between cataract mature senile and visual acuity in EKEK+ IOL and Phacoemulsification+IOL (p = 0.210) but there was a difference in visual acuity patient cataract matur senile between the first and seventh day postsurgery EKEK+IOL and Phacoemulsification+IOL (p = 0.000). Conclusion: There was no relationship between visual acuity improvement of patient mature senile cataract postsurgery and operative technique but improvement of visual acuity in mature senile cataract patients postsurgery Extraction Cataract Extra Capsular is 2 weeks slower than Phacoemulcification with Implantation Intraocular Lense.
