Background: Patients who use mechanical ventilator breathing devices lose the ability to remove secretions manually. This situation puts the risk of airway obstruction. Collaborative intervention that serves to prevent airway obstruction caused by secretions is suction (Price & Wilson, 2012). Aim: Describes the implementation of emergency nursing practice which is focused on the implementation of nursing care for patients with Respiratory Failure on Mechanical Ventilator with main nursing problems ineffective airway clearance and evidence-based review in the GICU RSUP Mohammad Hoesin Palembang. Method: The method of this research used a qualitative method with a case study approach. The subjects of this case study were three patients to explore the problem of nursing care for patients with respiratory failure in the GICU RSUP Mohammad Hoesin Palembang. Result: Based on the results of the assessment, it was found that the three patients had respiratory disorders, so they had to use a mechanical ventilator with SIMV and PS modes. The priority of nursing problems in the three patients was ineffective airway clearance which was characterized by excessive sputum production. After suction the patient looks calm because it reduces secretions. The nursing problem of airway clearance in the three patients has not been resolved so that interventions must still be carried out. Conclusion: The conclusion of the researcher is that all respondents who have airway trauma or airway obstruction and have decreased peripheral oxygen saturation must be given suction i there has criterias for suction action.
