KARAKTERISTIK SOSIODEMOGRAFI DAN KLINIS PASIEN KANKER PARU METASTASIS SISTEM SARAF PUSAT DI RSUP DR. MOHAMMAD HOESIN PALEMBANG PERIODE 2018-2020
Brain metastases is a 9 attachment malignancy of the central nervous system with a high mortality rate and an incidence that can reach 310 times more than primary brain tumors. Lung cancer is the main source of metastases for brain and spinal cord metastases. Central nervous system metast ases and lung cancer are multifactorial diseases that can be caused by old age, gender, genetics, smoking history, history of other lung diseases, history of extrapulmonary disease, and occupation. Lung cancer was divided into two groups based on the histo pathological findings, namely small cell lung carcinoma and nonsmall cell lung carcinoma. Lung cancer is usually diagnosed at an advanced stage due to clinical symptoms that are not typical at the beginning of the disease. The clinical symptoms of lung ca hemoptysis , dyspnea ncer are coughing, and chest pain. Lung cancer lesions can metastasize to either solitary or multiple lesions in the central nervous system. Clinical symptoms due to central nervous system metastatic lesions include pain, seizures, moto r deficits, sensory deficits, impaired cognitive function, and cranial nerve disorders. Management of lung cancer and central nervous system metastases consists of chemotherapy, radiotherapy, surgery and medication. Therefore, this study aims to determine the sociodemographic and clinical characteristics of lung cancer patients with metastases to the central nervous system at Dr. RSUP. Mohammad Hoesin Palembang. . The This research is an observational descriptive study with a cross sectional design approach sample in this study were all lung cancer patients diagnosed with metastases to the CNS (brain and spinal cord) recorded in the medical records of Dr. RSUP. Mohammad Hoesin Palembang who has met the inclusion and exclusion criteria with the total sampling method. A total of 41 samples of lung cancer patients . with central nervous system metastases were obtained in Dr. RSUP. Mohammad Hoesin Palembang. This study has the results that the and majority of resear average age of research subjects are 54 years old, ch subjects are patients aged 51 70 years (34.1%), male (73.2%), high school graduates (68.3%), and working as private employees (25, 8%). 26.8% of patients had a history of smoking, 14.6% of patients had a history of pulmonary and extrapulmonary disease , and all subjects had most clinical symptoms were cough (56.1%) and no family history. The headache/back pain and cranial nerve deficits (68.3%) with lung cancer type NSCLC (90.2%) whose diagnosis was made at an advanced stage (82.9%) by radiological exam ination and histopathology (48.8%) and chemotherapy as the main specific treatment (31.7%). Extracranial metastases were found in only 29.3% of patients. Lung cancer manifested as multiple lesions (70.7%) in the brain (75.6%) with synchronous time of onset (46.3%). The majority of patients were diagnosed with MRI (39%) and radiotherapy (24.4%) as specific treatment 40 (2013 of central nervous system metastases.
